Comprehensive OT Home Assessment
A whole-home review shaped around the person, daily routines and the parts of the property that matter most.

Homes Reimagined
A practical, person-centred home assessment for individuals, families and professionals who need clear recommendations about everyday safety, access and home adaptations.
Who it's for
This service is for people who want their home to better support daily life, whether the concern is recent, long-standing or part of planning ahead. It can also help relatives, carers, case managers and other professionals understand the practical questions to consider.
It is often useful before ordering a bathroom or kitchen, choosing an access solution, planning a return from hospital, or starting a larger adaptation. The assessment considers the person, their routines and the relevant parts of the home together.

See the assessment packageHow it works
Phase 01
We start with a discussion about everyday routines, priorities and the parts of home that feel difficult.
Phase 02
The Occupational Therapist reviews the relevant rooms, routes and practical factors in the property.
Phase 03
You receive written recommendations for the home, equipment or adaptation options discussed.
Falls & home safety
A fall, a near miss or simply losing confidence in a familiar task can prompt questions about the home. An assessment can consider the relevant spaces and routines, such as entrances, stairs, bathroom use, lighting, storage and movement between rooms.
DFG & council adaptations
The council route exists for home adaptations. A private report can support conversations by setting out needs and recommendations, but it does not guarantee funding or determine eligibility. The relevant local authority makes its own decisions about assessment, evidence and any grant.
Hospital discharge
A private assessment can help a household consider how the home may work after a hospital stay. It complements rather than replaces the discharge planning and decisions made by the hospital, local authority or other relevant services.
Assessments are led by Haela Pattemore, an HCPC-registered Occupational Therapist.

The fixed fee is £349 and includes the assessment and written report. If you would like to discuss whether this is the right starting point, you can arrange a free 15-minute telephone call.
No. You can contact Homes Reimagined directly to discuss a private OT home assessment. You may also ask a relative, carer or professional to join the conversation if that is helpful.
The time needed depends on the person, the home and the questions being considered. We will discuss the scope when you contact us, so you know what to expect before booking.
The Occupational Therapist talks through daily routines and priorities, considers the relevant areas of the property and discusses practical options. A written report records the recommendations discussed.
A private report can help explain needs and recommendations in conversations with a local authority. The council decides its own assessment process, evidence requirements, eligibility and any funding award.
A Disabled Facilities Grant is commonly means tested for adult applicants, but the rules and any contribution can depend on individual circumstances. The relevant council can explain its current process and how it applies to a particular case.
A private assessment can consider how the home may support someone returning from hospital. It does not replace the discharge process or decisions made by the hospital and relevant services, but it can help a household understand practical home considerations.
Please contact us for current availability. We will discuss the home, the type of assessment needed and any discharge date or urgency you would like us to be aware of.
Yes. Homes Reimagined's clinical lead, Haela Pattemore, is HCPC registered as an Occupational Therapist (OT73787).
